In order to register for direct billing with our clinic, please present your insurance benefits member ID card to the front desk upon arrival. You must be insured by one of the above listed carriers in order to qualify*.
*By signing up for direct billing, you are authorizing the insurer/plan administrator to issue payment directly to your provider/CMC. In the event your claim(s) are declined by the insurer/plan administrator, you will remain responsible for payment to your provider/CMC for all services rendered and/or supplies provided.
