All staff members who come in contact with your personal information are aware of the sensitive nature of the information that you have disclosed to us. They are trained in the appropriate use and protection of your information.
CMC's privacy protocols comply with Ontario privacy legislation and the standards of each of CMC's practitioners' regulatory bodies. In line with a collaborative health setting you are allowing relevant information shared during your appointments to be shared with other practitioner(s) at CMC, provided that your healthcare provider deems it beneficial to the treatment and has expressed this to the you, the patient. You are also authorizing the clinic and its associated healthcare professionals to communicate with your family doctor and/or other referring healthcare providers as deemed necessary for your beneficial treatment. Relevant information is considered any information that helps to drive treatment protocols in order to provide the best care for the patient.
You are authorizing CMC and its associated healthcare professionals to collect your personal and medical information as documented above. Storage, retention and destruction of your personal information are in compliance with existing legislation and privacy protecting protocols.
Your appointment time is reserved just for you. A late cancellation or missed visit leaves a hole in the therapists' day that could have been filled by another patient. As such, we require 12 hours notice for all Chiropractic services and 24 hours notice for all remaining services at CMC for any cancellations or changes to your appointment. Patients who provide less than the required notice will be charged accordingly:
a) 40% of the service fee for all Chiropractic services.
b) 50% of the service fee for all other services provided by CMC.
Appointments are scheduled for a specific amount of time. In the event that you are late to an appointment, the service will be only rendered until the end of the scheduled appointment time and the full fee for the service that was booked will be due. If you arrive after the end of the scheduled time slot it will be considered a missed appointment and charged accordingly.
Missed Appointments:
Patients who miss their appointments without notice will be charged up to 50% of their scheduled appointment fee. If a valid credit card is on file it will be charge the same day. If a valid credit card is not on file an invoice will be sent to the patient, to be paid within 14 days.
Some of our healthcare providers may be legally required to do a reassessment of your condition if a significant amount of time has passed since your last appointment. In these situations the doctor/therapist has a professional obligation to reassess your health in order to provide the best care possible. Reassessment fees are outlined in the clinic fee schedule and may take longer than a regular treatment time.
You will automatically be booked for a reassessment with your doctor/therapist if you have not been in for care for that particular service in the following time periods:
Chiropractic: 6 months - re-assessment
Naturopathic Medicine: 1 year - initial consultation
Homeopathy: 6 months - initial consultation
Patients are responsible for all charges rendered and payment is expected to be made in full on the day service. We accept Cash, Debit, Visa, MasterCard, Amex, Google Pay and Apple Pay. If a patient is approved to run a tab, they must leave a valid credit card on file. The full amount of the patient's account will be charged to their card on the first Friday after their last visit, in the case that no further visits are scheduled. An account may not accrue charges for longer than 14 days at a time and will be charged accordingly.
All services fees are available in person at the clinic or online at mobilitychiro.ca. All of CMC's fees are subject to change without notice to clients.
Appointment reminders are available through either email notification or text message. These notifications can be managed through your personal profile on the Jane app, or you may ask the front desk to set notifications at any time. Appointment reminders are solely a courtesy and you are fully responsible for your booked appointment time regardless of whether or not you receive an appointment reminder.
As a patient of CMC, you will occasionally receive emails regarding clinic updates such as closures, holiday hours and clinic related news. You are able to unsubscribe at any time.
Due to the nature of items sold at, by or through Complete Mobility Chiropractic, any open or used products may not be returned under any circumstance. This includes but is not limited to: vitamins, supplements and nutraceuticals, topical treatments, pillows, exercise equipment, braces, kinesiology tape and MediStik products.
Products that have not been opened or used may be returned for a clinic credit, within 14 days of purchase and with the original receipt displaying the purchase. Returns are subject to staff consideration as to whether the product has been opened, used, or otherwise affected.
Special order products, including massage equipment and heating pads, are non-refundable.
Custom orthotics and Homeopathic remedies are non-refundable.
Any and all persons who interact with Complete Mobility Chiropractic, its associates/service providers or its patrons are subject to the following Zero Tolerance Policy. This includes interactions via e-mail, website, social media, direct messaging, text/phone and/or in person both on and off clinic property. By accessing this website or contacting our clinic in any capacity, all persons agree to be bound by the disclaimer as outlined by the following policy:
Violent, abusive, or aggressive behaviour in any capacity, including unsolicited/unwanted sexual advances, will not be tolerated.
Such behaviours are deemed unacceptable and include, but are not limited to the following:
A breach of the Zero Tolerance policy constitutes grounds for Complete Mobility Chiropractic, including its associates/service provides, to decline and/or discontinue services effective immediately. Appropriate referrals will be made for continuity of care.
